House Book Template — Free Download

Under the Czech Foreigners’ Residence Act (§ 100(f) and § 101 of Act No. 326/1999 Coll.), the house book is the register of accommodated foreign nationals, EU citizens included. It holds the data from the foreigner’s registration form (§ 97: name, date of birth, citizenship, permanent address abroad, travel document number, visa number if one is shown in the document, start, place, expected length and purpose of stay) plus the start and end of the stay, and is kept for 6 years from the last entry. The law does not require Czech guests to be entered in it. The register of every paying guest, Czech guests included, is a separate one: the evidenční kniha for the tourist fee (§ 3g of Act No. 565/1990 Coll.), kept where the municipality charges the fee, which also records the identity document type and number and the fee collected or the reason for exemption.

This PDF template is meant to be printed and filled in by hand and has columns for both registers: a header for the property’s name and address and the sheet number, then rows for a sequence number, surname, first name, citizenship, purpose of stay (medical/recreational, business, educational, other), identity document type and number, visa number, permanent address (country, municipality, district, street and house number), arrival, departure and birth dates, the tourist fee collected or the exemption reason, the guest’s signature, and a notes field.

The form’s field labels are in Czech, since that’s the language local authorities and the Foreign Police expect when they inspect the register. It’s meant as a ready-to-use form for small operators who still keep records by hand. The law doesn’t prescribe the layout, but both acts list exactly which data the registers must contain. The Foreign Police state on their website that the house book is kept and presented in paper form for inspections, so if you keep records electronically, be ready to print them; the tourist-fee register may expressly be kept on paper or electronically (§ 3g(1)).
